Smart Choice Plan Care provides support coordination to NDIS self-managed and plan-managed participants. Our team consists of staff who have received the right training and have excellent skills. We thrive to deliver our best service to get the maximum benefit from your NDIS plan. You have the choice and control of your plan and services. Also, you have the freedom to select any service providers of your choice.
Our Services
Our support coordinators will work with you and help you to find most suitable providers, products and services for your needs.We negotiate prices and services as part of any quotable budget, create a service agreement and service bookings with preferred providers of your choice. We work with the budget for each support category and advise to your plan manager as you start your NDIS Journey.We will provide our assistance in resolving any issues that may occur and offer supports in understanding their rights and responsibilities under the service agreements and provide assistance when termination or any changes to their service agreement has been made.
Smart Choice Plan Care offers Level 2 Support Coordination.
Having a Support Coordinator can help to make it easier to connect with providers and the local community to help achieve personal goals. We value honest communication and work hard to find the best quality services that provide participants with the highest standard of care.
We support NDIS Participants to:
- Help you understand how the plan works and what is available to you
- Support you in making decisions and exercise choice and control over your plan
- Assist with negotiating with providers about their services and fees to ensure you reach your goals
- Develop service agreements and bookings with your providers
- Coordinate supports and help you access your local community
